What is a Certificate of Insurance?

A Certificate of Insurance is a document issued by an insurance companies to provide evidence of insurance and information on specific insurance coverage. The Certificate usually includes the name of the insured, information on types and limits of coverage, and policy numbers and effective dates. Certificates are usually requested by an agreement, contract, or transaction to make certain each party has the appropriate insurance coverage.

Certificates Issued by the College

The College may be asked to issue a Certificate of Insurance to satisfy a contract requirement (e.g. rental venue). A request containing the following information regarding the party requesting the Certificate should be e-mailed to certificates@lafayette.edu:

  • Name
  • E-mail Address
  • Physical Address
  • Date of Event
  • College Contact
  • Type of Insurance to be Verified
  • Whether Additional Insured Status is Needed

It is also helpful to provide electronic copies of the insurance paragraphs of the contract requiring the Certificate.

 

Certificates Requested by the College

To manage liability exposures, the College requires that parties using College facilities and companies working for the College or on the College’s campus provide a Certificate of Insurance. Such parties include, but are not limited to:

 

Third Party Using College Facilities

A Certificate of Insurance evidencing a minimum o9f $1,000,000 bodily injury and property damage combined liability limit per occurrence from the third party’s insurance agent must be provided to the Scheduling Office http://scheduling.lafayette.edu/.  Lafayette College must be named as an Additional Insured on the General Liability policy. The Certificate must specifically state that the Third Party’s insurer acknowledges that the College’s coverage as an Additional Insured includes the defense and indemnification for claims and damages against Lafayette that are attributable to allegations of negligent acts or omissions on the part of Lafayette or its employees or agents.

 

This Certificate must clearly show that the coverage period includes the date(s) of the third party’s event.  If the event includes participation by minors (i.e. summer camps), the Certificate must clearly state that sexual abuse and molestation coverage is included in the policy.

 

Vendors Providing Goods & Services to the College

The Vendor’s insurance company must provide Certificate of Insurance to Lafayette College (Facilities Operations, 9 North Campus Lane, Easton, PA 18042) that reflect the following coverages:

1)   General liability insurance with a minimum combined single limit of:

o    $1,000,000 Each Occurrence for contracts/purchase orders valued at less than $1,000,000.

o    $5,000,000 Each Occurrence for contracts/purchase orders valued at greater than $5,000,000.

Lafayette College must be named as an Additional Insured on the General Liability policy. The Certificate must specifically state that the Vendor’s insurer acknowledges that the College’s coverage as an Additional Insured includes the defense and indemnification for claims and damages against Lafayette that are attributable to allegations of negligent acts or omissions on the part of Lafayette or its employees or agents.

2)   Automobile property and liability insurance coverage; and

3)   Workers Compensation (WC) and Employers’ Liability stating that the WC Statutory Limits have been met by your policy.  Either the WC Statutory Limits box must be checked or “Complies with PA Statutory Limits” must be specifically stated.

 

Contractors

The Contractor’s insurance company must provide Certificate of Insurance to Lafayette College (Facilities Operations, 9 North Campus Lane, Easton, PA 18042) that reflect the following coverages:

  1. General liability insurance with a minimum combined single limit of:
    • $1,000,000 Each Occurrence for contracts valued at less than $1,000,000.
    • $5,000,000 Each Occurrence for contracts valued at greater than $5,000,000.
    • $10,000,000 Each Occurrence for contracts valued at greater than $10,000,000.

Lafayette College must be named as an Additional Insured on the General Liability policy. The Certificate must specifically state that the Contractor’s insurer acknowledges that the College’s coverage as an Additional Insured includes the defense and indemnification for claims and damages against Lafayette that are attributable to allegations of negligent acts or omissions on the part of Lafayette or its employees or agents.

 

In addition, evidence that the “Additional Insured – Owners, Lessees or Contractors Completed Operations” form (Form CG2037 10 01 or its equivalent) covering Lafayette College in the next two renewals must be documented;

  1. Automobile property and liability insurance coverage; and
  2. Workers Compensation (WC) and Employers’ Liability stating that the WC Statutory Limits have been met by your policy. Either the WC Statutory Limits box must be checked or “Complies with PA Statutory Limits” must be specifically stated.
