Report a Lost Key or FOB

*There is a fee associated to replace any key or Pard Card.

Please contact Public Safety IMMEDIATELY if you have lost your room key or dorm key.

FACULTY and STAFF

Please Note: If there is NO immediate security threat and you need to replace a Physical Key (s) ONLY! Please follow the lost key SOP listed below.

1. The Department Head / Supervisor must be notified and; send a notification, e-mail is acceptable, to the Director of Public Safety: Jeffrey Troxell, (troxellj@lafayette.edu) copies to the Director of Facilities Operations: Scott Kennedy (kennedsc@lafayette.edu) and the Access Control Office: Roy Bollinger, Ken Murray, Steve Kauth, Nate Breiner (accesscontroloffice@lafayette.edu).
2. In the notification, please tell us what is missing, keys numbers if known, and what areas /building and room numbers are affected.
3. Tell us the date and circumstances surrounding their loss.
4. Please advise us whether or not you believe there is a security threat involved with the loss, and what action you believe should be taken.
5. If the person needs replacement key(s), please ask that they be replaced ASAP.
